CDC Testing Information for PPM Labs - Booklets

This booklet is downloadable from the CDC website and has a lot of useful considerations regarding microscopy testing.

Included in each booklet are forms for various purposes such as safety, QC, QA, etc.

Examples of how to fill in each form precedes a blank form that the user can print and use.

The BLANK FORMS have been edited to allow the user to type in the header information, as it is a requirement that each form contains the name of the laboratory.

The blank forms in this booklet have been modified to allow the user to type in the Name & Location of the Laboratory.

Do this once on the form on page-28 - and headers on the other blank forms in this booklet will fill in automatically.

Some other fields will also allow typing in text.

Be sure to save your modified booklet, and instruct users on how to print forms as they are needed.
