CDC Testing Information for Waived Labs - Booklets

These booklets downloadable from the CDC website have a lot of useful considerations regarding laboratory testing.

Included in each booklet are forms for various purposes such as safety, QC, QA, etc.

Examples of how to fill in each form precedes a blank form that the user can print and use.

The BLANK FORMS have been edited to allow the user to type in the header information, as it is a requirement that each form contains the name of the laboratory.
